Patient Portal

Happy new year!  We are pleased to share we have launched a new patient portal called Patient Passport by Elation. 

 

To activate the new portal please call the office, email info@efhwc.com OR make the change at your next appointment. Once you have set up your account you will be able to login to your passport using the portal button below.

Our patient portal is a secure online website that gives you convenient 24-hour access to your personal health information and medical records – called an Electronic Health Record or EHR – from anywhere with an internet connection. 

​

Accessing your personal medical records through the patient portal can help you be more actively involved in your own health care. 

​

The portal is intended for non-urgent messages, questions to staff or your provider.  Replies to messages can take up to 72 hours.  

​

If you require more urgent attention, please call the office. 

​

Refill requests should be requested through your pharmacy.
